2020-2021 LAUSD and Canyon Opening Packet Forms
For safety and compliance, it is critical that the office receives all of the required items listed below filled out with parent/guardian signature no later than Thursday, September 3rd. Please notify the staff in the main office if you have any questions about completing these forms and we will be happy to assist you. You may reach the front office by calling (310) 454-7510.
These forms must be completed annually. Note: these forms are only required for currently enrolled students. If you are hoping to enroll your child at Canyon, please view our enrollment page HERE.
Please print out the below checklist and check off the forms as you complete them. Please be sure to indicate your child's name on each of the forms. Many of these forms are designed to be completed online and submitted to the office. You may also print/scan and send to jaclyn.arabia@lausd.net
